ONLINE EVENT REGISTRATION
How to Pre-Register 1-2-3 ONLINE!
Registrations can be done the day of the event, but why wait!?
Pre-registration will save you some time the day of the event, give you a reduced registration fee ($20 per person instead of $25) and will entitle you to $10 worth of free raffle tickets as a thank you!

1. Read Event Details and Waiver/Release statement.
2. Submit your registration information on this page. Please be sure to include all participants who will be joining you. Kids under 10 are free!
You can use the PayPal link to make one payment for those you are registering as long as you specify on the notes section of your payment.)
3. DONATE your $20 per person registration fee via PayPal (which accepts all major credit cards) using the same name on the registration form. You can make one payment for all of those you are registering - please specify on the notes section of your payment. PLEASE NOTE ON YOUR PAYMENT THAT THIS IS FOR A REGISTRATION FEE PAYMENT.
Registration Fee entitles you to:
- Participation in our 5th Annual Torbethon 5K and Family Fun Fair
- $10 worth of raffle tickets
- Free DTL Merchandise
MUST REGISTER NO LATER THAN SEPTEMBER 14, 2014 TO QUALIFY FOR PRE-REGISTRATION

Once your form and payment have been received, you will receive a email to confirm your registration.

Waiver/Release:
In consideration of this entry being accepted, I herby waive and release claims against the Daniel T. Liss Memorial Fund and Bridgewater Township, staffs, officers, volunteers, sponsors, successors or assigns for injuries that I may suffer at this event. Further, I grant the right to use my likeness in any photographic record of the Daniel T. Liss Memorial Fund, 5th Annual Torebethon. I further attest that I am medically and physically able to participate in said event.
Electronic Signature confirms acceptance of Waiver/Release statement. A copy of the waiver verbiage can be sent to participants via email upon request.
