How to Pre-Register 1-2-3!

Registrations can be done the day of the event.  Pre-registration will just save you some time the day of and will entitle you to one extra sheet of primary level raffle tickets as a thank you! You can also print the form and bring it with you that day to complete your registration.



1.  PRINT the form on this page and complete it.





2.  PAY with a check, payable to the Daniel T. Liss Memorial Fund, fee is $25 PER participant.  Your fee entitles you to 

-Participation in our 5K Walk or Run

-DTL Tee Shirt

-$10 worth of Primary Level raffle tickets
